We are currently seeking an experienced and highly skilled Advanced Nurse Practitioner to join our team in providing exceptional care to residents in our care homes. As an Advanced Nurse Practitioner, you will play a pivotal role in delivering comprehensive healthcare services, including assessment, diagnosis, treatment, and management of residents' healthcare needs. Your advanced knowledge and clinical expertise will contribute to the overall well-being and quality of care for our residents.


Responsibilities:


  • Comprehensive Assessment: Conduct comprehensive health assessments, including physical, psychosocial, and functional assessments, to identify residents' healthcare needs and develop personalized care plans.

  • Clinical Decision-Making: Utilize advanced clinical judgment and critical thinking skills to diagnose and treat common acute and chronic illnesses. Prescribe medications and initiate treatments based on evidence-based practice guidelines.

  • Collaborative Care Planning: Collaborate with the interdisciplinary team, including physicians, nurses, therapists, and caregivers, to develop and implement individualized care plans. Coordinate care and ensure effective communication among team members.

  • Chronic Disease Management: Manage and monitor residents with chronic diseases, such as diabetes, hypertension, and respiratory conditions. Provide education, support, and counseling to residents and their families for self-management and prevention of complications.

  • Medication Management: Prescribe, monitor, and adjust medications as needed. Educate residents on proper medication administration, potential side effects, and interactions. Collaborate with pharmacists to optimize medication regimens.

  • Health Promotion and Education: Promote residents' health and well-being through health promotion activities, preventive screenings, and health education. Provide counseling on lifestyle modifications, disease prevention, and health maintenance.

  • Advanced Procedures: Perform advanced nursing procedures within your scope of practice, such as wound care, minor surgical procedures, and administering injections. Ensure proper techniques, infection control, and patient safety.

  • Emergency Response: Assess and manage emergency situations effectively, providing immediate care and coordinating appropriate interventions. Stabilize residents and arrange for necessary medical interventions or transfers to higher levels of care if required.

  • Quality Improvement: Participate in quality improvement initiatives, monitor outcomes, and contribute to the development and implementation of best practices in care delivery. Continuously evaluate and improve resident care outcomes.

  • Professional Development: Engage in ongoing professional development activities to enhance your knowledge and skills. Stay updated with current research, evidence-based practices, and advancements in advanced nursing practice.



Requirements:



  • Education and Licensure: Hold a valid and current Advanced Nurse Practitioner license in the relevant jurisdiction. Graduation from an accredited advanced nursing program with a Master's or Doctoral degree is required.

  • Clinical Expertise: Possess advanced clinical knowledge and skills across a wide range of healthcare areas. Previous experience as an Advanced Nurse Practitioner in a care home or primary care setting is preferred.

  • Autonomous Practice: Demonstrate the ability to work autonomously, make independent clinical decisions, and manage complex healthcare conditions. Maintain accountability for your practice and adhere to regulatory standards and ethical guidelines.

  • Critical Thinking and Problem-Solving: Exhibit excellent critical thinking, assessment, and diagnostic skills. Apply evidence-based practice in clinical decision-making and problem-solving to ensure optimal resident outcomes.

  • Communication and Interpersonal Skills: Communicate effectively and compassionately with residents, families, and the healthcare team. Build trust and establish therapeutic relationships. Collaborate and work well within a multidisciplinary team.

  • Leadership and Mentorship: Demonstrate leadership skills, including the ability to mentor and support other healthcare professionals. Contribute to the professional development of colleagues and promote a culture of continuous learning.

  • Adaptability and Flexibility: Adapt to changing priorities and healthcare environments. Be flexible in meeting the evolving needs of residents, the care home, and the healthcare team.

  • Technological Proficiency: Possess proficiency in electronic health record systems and other healthcare technologies. Utilize technology to enhance the delivery of care, documentation, and communication.

  • Professionalism and Ethics: Uphold professional standards and ethical practice in all aspects of care. Maintain confidentiality and advocate for residents' rights and well-being.
